The majority of re-hospitalizations can be prevented by the implementing the doctors’ orders. Our care team put the following systems in place.

The Hospital Visit The service starts at the hospital, where we visit your loved one and work with their doctors to ensure we have all the orders and requirements. This will ensure our care team have a clear understanding of the post-op instructions, and any dietary restrictions.
The Home When your loved one is discharged, the team which includes the caregiver, nurse and internal case manager implements the doctors orders to create a individualized care plan in the comfort of their home. The family can contact us at anytime with questions or updates regarding your love one care and services.
